Application Procedures
Step 1:
1.Please download and complete OVGA Junior Camp Application Form.
2.Download and complete Release and Waiver of Liability and Indemnity Agreement.
3.Download and complete Media Waivers.
4.Immunization Record File & Health Information Form.
5.50% tuition pay (Check payable to Oak Valley Golf Academy or fill out Credit Card Authorization Form). The deposit is refundable only if the student is not accept or in the case of Visa is denied.
6.Non-refundable $300 application fee.
7.2 passport size photos.
8.Please return the documents to the address below:

Step 2:
Once you have been approved for the admission, Oak Valley Golf Academy will issue an Acceptance Letter to you for your US Visa Application.
Once you receive the Acceptance Letter, you can make an appointment from the US Embassy/Consulate in your country for your visitor’s visa (B-1/B-2).

Step 3:
You will have to register with Oak Valley Golf Academy when you arrive the United States no later than the expected date. The followings are needed to complete the registration:
1.Copy of I-94
2.Copy of Passport
3.Balance pay in full
You will also receive ONE Oak Valley shirt and ONE Oak Valley logo hat when you register. Enjoy your time at Oak Valley!
